SMITHFIELD, N.C. – A medical examiner's report shows a four-year-old Smithfield girl died of blunt force head injuries in July.

Teghan Skiba died July 19 at UNC Hospitals. Her mother's boyfriend, Jonathan Richardson, faces charges of first-degree murder in connection with Skiba's death.

Investigators say Richardson "lost it" and beat Skiba with a cord after she soiled the bed they were sleeping in. Investigators also found human bite marks, bruises, cuts and signs of sexual abuse.

Skiba's mother, Helen Reyes, faces negligent child abuse charges. She is out of jail on bond.

Richardson remains in jail under a $1 million bond.
